"Muppet" has been an insult for as long as I can remember; I don't know whether it predates The Muppet Show or not as that kind of coincides with my earliest memories, but I've certainly heard it used in a derogatory sense for the past 20 years.
Likewise. I think it's mostly a British insult, though.
